It substantiates that the effectiveness of PPP projects depends on the balance of interests between the state and the private sector, the coherence of their interaction, and the rational distribution of managerial functions throughout all stages of the project life cycle–from initiation to operation. It is established that a key factor in achieving project success is the presence of a clearly structured management model that ensures coordination, monitoring, and control within a continuous feedback framework. The article reveals the specifics of building organizational structures for PPP management, defining the functions of public authorities, private partners, independent consultants, and coordinating institutions. It is proved that transparency of management procedures and effective communication between the participants of a partnership form the foundation of mutual trust, which, in turn, strengthens the sustainability of the overall governance system. In the context of ongoing digital transformation, an essential element of management mechanisms is the use of information and analytical platforms for risk assessment, performance forecasting, and real-time decision adjustment. The study emphasizes the importance of integrating strategic, operational, and financial management within a unified project governance system, which enhances flexibility and adaptability under crisis conditions. It also substantiates the feasibility of establishing a national coordination center for healthcare PPP projects to provide methodological, legal, and analytical support for partnership initiatives. The findings confirm that the implementation of effective organizational and managerial mechanisms contributes to improving the quality of healthcare services, optimizing expenditures, reducing financial risks, and forming a results-oriented management system based on social value. The proposed concept integrates the principles of value for money, responsible governance, and innovation, ensuring the long-term sustainability of partnership models and facilitating the modernization of Ukraine’s healthcare system amid post-war recovery and European integration transformations.У статті здійснено комплексне дослідження організаційно-управлінських механізмів реалізації проєктів публічно-приватного партнерства (ППП) у медичній сфері як складової системи стратегічного управління охороною здоров’я. Обґрунтовано, що ефективність ППП визначається рівнем збалансованості інтересів держави та бізнесу, гармонійною взаємодією між секторами й раціональним розподілом управлінських функцій на всіх етапах життєвого циклу – від ініціювання до експлуатації об’єкта. Встановлено, що ключовим чинником результативності партнерських проєктів є наявність чітко структурованої моделі управління, яка забезпечує координацію, моніторинг і контроль у режимі безперервного зворотного зв’язку. Розкрито специфіку побудови організаційних структур управління ППП, зокрема визначено функції державного замовника, приватного партнера, незалежних консультантів та координуючих органів. Доведено, що прозорість управлінських процедур і комунікаційна взаємодія між суб’єктами партнерства є основою довіри, що формує стійкість усієї системи управління. Зазначено, що в сучасних умовах цифровізації важливою складовою управлінських механізмів стає використання інформаційно-аналітичних платформ для оцінки ризиків, прогнозування ефективності та коригування управлінських рішень у режимі реального часу. Акцентовано на необхідності інтеграції стратегічного, операційного та фінансового менеджменту у межах єдиної системи управління проєктами, що дозволяє досягати більшої гнучкості й адаптивності у кризових умовах. Обґрунтовано доцільність створення національного центру координації ППП-проєктів у сфері охорони здоров’я, який би забезпечував методичну, правову та аналітичну підтримку партнерських ініціатив. Результати дослідження підтверджують, що впровадження ефективних організаційно-управлінських механізмів сприяє підвищенню якості медичних послуг, оптимізації витрат, зменшенню фінансових ризиків та формуванню системи управління, орієнтованої на результат і суспільну цінність.
